Our process begins with a pre-admission assessment for each referral. After it is determined that we can meet the needs of a resident, we move to the development of their Individualized Service Plan (ISP). We utilize the ISP to continually evaluate, enhance and maximize their potential for daily living. Each month we will implement goals for their residents while participating in goal activities and utilize a plan of action to track their progress.

Our services include the following:

  • Health monitoring and assessments through the Owner and House Coordinator (A Registered Nurse Consultant will be utilized on case by case as see fit)
  • Assistant with daily activities
  • Medication Management by certified and trained staff
  • Laundry
  • Recreation events
  • Assistance with showers and baths
  • Assistance with personal funds by using our Resident Trust Account
  • Coordination of transportation and medical appointments
  • On-site (if needed) barber and beautician services
  • Basic cable television and telephone services
  • Individual therapeutic support
  • Planned and resident choice meals
  • Most of the services are included in your fees, but some are subjected to third party billing. This is covered in more detail in our resident service agreement.

The following employees are available:

  • Facilities Director/Owner: Full time and on call 7 days a week, 24 hours a day.
  • House Coordinator: Full time and on call 7 days a week, 24 hours a day.
  • Personal Care Worker (PCW): 7 days a week, 24 hours a day as needed.
  • Maintenance Technicians: As needed.
  • Registered Nurse Consultants: 24 hour on call coverage.
